Building Bridges to Treatment<\/p>\n

\u00bb THE RISE Program is offered by Ventura County Behavioral Health specifically to encourage and enable people in these situations to get assessment and treatment. The field-based outreach team makes contact, then provides ongoing support in navigating any challenges to accessing care. And it\u2019s not just one-time assistance \u2013 the RISE team follows up with clients as needed and may be closely involved with case management.<\/p>\n

\n3) Email the referral form: Riseprogram@ventura.org<\/a><\/strong><\/p>\n

If someone has, or appears to have mental health problems and is unable or unwilling to access assessment and treatment, RISE can help.<\/p>\n

DO YOU KNOW SOMEONE …
\n… who has been released from a psychiatric facility but isn\u2019t following up with outpatient treatment?
\n… who is homeless and has a mental illness but currently refuses treatment?
\n… with serious mental health problems who is unable to access outpatient mental health clinics without assistance?
\n… whose symptoms are so severe that they cannot leave their home?
\n… who is having difficulty managing their symptoms in a public setting?
\n… who appears to be experiencing a low level crisis due to mental illness and is impacting the community?<\/p>\n

If a person is experiencing a moderate or high risk crisis \u2013 if their words or behaviors suggest they may harm themselves or others, or if they appear to be gravely disabled \u2013 do not contact RISE.
